We are writing to make you aware of an important provider change at our West Creek location.

Dr. Claire Thomas will be leaving employment with us at Commonwealth Primary Care, Inc. Her last day in the office will be Friday December 15, 2023. We have enjoyed having her as part of the CPC team and wish her well. Dr. Thomas has expressed that she greatly enjoyed the patient relationships she formed while here and will miss them along with the CPC team.

If you are a patient of Dr. Thomas and have an appointment scheduled with her after Friday December 15, 2023 our office will contact you soon to reschedule with any of our other providers or feel free to call (804) 288-0057 to reschedule your appointment at your convenience.
