It is with mixed emotions that I announce my retirement on June 30, 2023, after 33 years of medical practice. Thank you so much for allowing me the great honor and privilege to be a part of your medical care. I look forward to spending more quality time with my wife, family, and friends.

To aid in this transition, Commonwealth Primary Care has many quality providers both here at the Glen Forest office and at our other CPC locations. I have complete confidence in all of them to provide excellent quality care for your future health care needs. A complete listing of Physicians, Physician Assistants, and Nurse Practitioners, as well as our multiple locations is available on our website www.cpcva.com.

To make an appointment with another CPC provider, please call our office and the staff will help you transition to a new provider at the location of your choice. Within CPC, your electronic medical records are readily available to them. If you wish to transfer your care to a physician who is not part of CPC, your medical records will be sent to your new provider after you fill out and sign the transfer of records form which is available at any CPC office or can be printed from our website.
