Your insurance contract is an agreement between you and your insurance carrier. Commonwealth Primary Care participates with most major carriers. As a service to you, we will file your claims with your insurance carrier.
As required by insurance carriers, you are responsible for the payment of deductibles, copays and non-covered services at the time of your visit. For your convenience we accept cash, check, Visa and MasterCard for payment.
You may receive a separate bill for lab work. If you have any concerns or questions regarding bills from other providers, please contact them directly.
Commonwealth Primary Care charges a fee for the completion of medical forms, insurance underwriting applications and life insurance applications.
Before your first appointment we suggest that you contact your insurance carrier to confirm that Commonwealth Primary Care is a participating provider.
Contact your insurance carrier before an annual complete physical exam to verify coverage.

We do not participate with most Medicare Advantage plans and Medicare HMO’s.
